Redefining sexual health for benefits throughout life


Systematically collecting and analyzing data disaggregated by sex and additional factors such as age, ethnicity, socio-economic status and disability, is critical. Harmful gender norms – including those related to rigid notions of masculinity – affect the health and well-being of boys and men. For example, notions of masculinity encourage boys and men to smoke, take sexual and other health risks, misuse alcohol and not seek help or health care. Such gender norms also contribute to boys and men perpetrating violence against women and girls. They also contribute to violence perpetrated against men including homicide, youth and gang violence, which are among leading causes of morbidity and mortality among young men. Data show that men’s increased risk of acquiring SARS-COV2, is also linked to their lower rates of handwashing, higher rates of smoking and alcohol misuse and, related to that – higher comorbidities for severe COVID-19 symptoms as compared to women.
By providing the latest evidence-based definitions, WHO is facilitating the diagnosis and appropriate management for a wide variety of conditions related to sexual health. Ahead of Valentine’s Day (14th February, 2022), a new analysis was published in the open-access journal PLOS ONE on the need to consider sexual pleasure, not only risk of disease, in designing sexual health programmes. Sexual health is a state of physical, emotional, mental and social well-being related to sexuality; it is not merely the absence of disease, dysfunction or infirmity.

Important pregnancy tests and checks


For example, indigenous women have worse maternal health outcomes than non-indigenous women and are less likely to benefit from health care services in Latin America and the Caribbean. Therefore, inequities in maternal health between different ethnic groups should be monitored to identify critical, modifiable, health system and community factors that could limit health care coverage, including language, religion, territory and place of residence. Monitoring health inequities is essential for designing more effective programmes and policies to reduce health risks among indigenous women[2]. Comprehensive sexuality education (CSE) is a curriculum-based process of teaching and learning about the cognitive, emotional, physical and social aspects of sexuality. WHO recently collaborated with partners, including HRP, to develop guidance for out-of-school CSE programmes that are appropriate and safe for different groups of children and young people. This complements the guidance developed by the United Nations on school-based sexuality education.

Prevention of STIs

WHO has been working in the area of sexual health since at least 1974, when the deliberations of an expert committee resulted in the publication of a technical report entitled "Education and treatment in human sexuality" (WHO, 1975). Sex is often categorized as females and males, but there are variations of sex characteristics called intersex. The term ‘intersex’ is used as an umbrella term for individuals born with natural variations in biological or physiological characteristics (including sexual anatomy, reproductive organs and/or chromosomal patterns) that do not fit traditional definitions of male or female[1].
On sexuality education, as with all other issues, WHO provides guidance for policies and programmes based on extensive research evidence and programmatic experience. There is sound evidence that unequal gender norms begin early in life, with harmful impacts on both males and females. It is estimated that 18%, or almost 1 in 5 girls worldwide, have experienced child sexual abuse. Sexuality education equips children and young people with the knowledge, skills, attitudes and values that help them to protect their health, develop respectful social and sexual relationships, make responsible choices and understand and protect the rights of others. A major milestone is the new edition of the International Classification of Diseases (ICD) which has a chapter on sexual health for the first time.
This not only contributes to improved health outcomes, but also to gender equality and wider development. By providing children and young people with adequate knowledge about their rights, and what is and is not acceptable behaviour, sexuality education makes them less vulnerable to abuse. The UN’s international guidance calls for children between the age of 5 and 8 years to recognize bullying and violence, and understand that these are wrong. It calls for children aged 12–15 years to be made aware that sexual abuse, sexual assault, intimate partner violence and bullying are a violation of human rights and are never the victim’s fault. Finally, it calls for older adolescents – those aged 15–18 – to be taught that consent is critical for a positive sexual relationship with a partner.
Several other rapid tests are under development and have the potential to improve STI diagnosis and treatment, especially in resource-limited settings. Other biomedical interventions to prevent some STIs include adult voluntary medical male circumcision, microbicides, and partner treatment. There are ongoing trials to evaluate the benefit of pre- and post-exposure prophylaxis of STIs and their potential safety weighed with antimicrobial resistance (AMR). In addition, emerging outbreaks of new infections that can be acquired by sexual contact such as mpox, Shigella sonnei, Neisseria meningitidis, Ebola and Zika, as well as re-emergence of neglected STIs such as lymphogranuloma venereum. These herald increasing challenges in the provision of adequate services for STIs prevention and control.
Infants are generally assigned the sex of male or female at birth based on the appearance of their external anatomy/genitalia. Given the many evidence gaps for achieving universal access to STI/HIV services, WHO is currently prioritizing a research agenda for improving the implementation of national STI programmes. This agenda will complement WHO’s leadership in developing  innovative point of care tests for quicker and more accurate diagnostic testing, and in specifying the "Global STI Vaccine Roadmap" to guide research and development for new vaccines against STIs. To eliminate diseases that affect sexual health, WHO is developing new global strategies to address STIs, including HIV – while taking into account the current pandemic-induced health system disruptions. Accurate diagnostic tests for STIs (using molecular technology) are widely used in high-income countries. However, they are largely unavailable in low- and middle-income countries (LMICs) for chlamydia and gonorrhoea.
