To minimize orthostatic hypotension, especially after the first dose, therapy should be initiated at the lowest dose and taken at bedtime. Sustained-release formulations (e.g., doxazosin GITS) are available and provide more stable plasma levels, reducing the risk of first-dose syncope.


Adverse Effects



The most common adverse effects are related to vasodilation and include dizziness, headache, fatigue, somnolence, and peripheral edema. Orthostatic hypotension, manifested as lightheadedness upon standing, is a notable effect especially after the first dose or after dose escalation. This risk can be mitigated by starting with a low dose and titrating slowly. Nasal congestion, dry mouth, and blurred vision occur less frequently. Compared to non-selective alpha-blockers, doxazosin has a lower incidence of reflex tachycardia due to its alpha-1 selectivity.


Rare but serious adverse effects include priapism (prolonged erection), intraoperative floppy iris syndrome (IFIS) in patients undergoing cataract surgery, and allergic reactions. Doxazosin should be used with caution in patients with a history of syncope or orthostatic hypotension. In large clinical trials, doxazosin was associated with a higher rate of heart failure compared to diuretics, leading to a black box warning for hypertension monotherapy in some contexts.


Drug Interactions



Doxazosin is metabolized by CYP3A4, so inhibitors of this enzyme (e.g., ketoconazole, itraconazole, clarithromycin, grapefruit juice) can increase its plasma concentrations and prolong effects. Inducers (e.g., rifampin, carbamazepine) may reduce efficacy. Concurrent use with other antihypertensives, especially vasodilators or diuretics, can potentiate hypotensive effects. Phosphodiesterase-5 inhibitors (e.g., sildenafil, tadalafil) used for erectile dysfunction can cause additive vasodilation and should be co-administered with caution to avoid severe hypotension. Alcohol can exacerbate the hypotensive effects.


Special Populations



Elderly: Older adults are more sensitive to the hypotensive effects and are at increased risk of falls due to orthostatic hypotension. Dose titration should be more conservative. Studies indicate that doxazosin remains effective for BPH in elderly men with no significant increase in adverse events when dosed appropriately.


Pregnancy and Lactation: Doxazosin is classified as Pregnancy Category C. There are no adequate well-controlled studies in pregnant women. It should only be used if the potential benefit justifies the risk. It is excreted in breast milk in small amounts; caution is advised for nursing mothers.


Hepatic Impairment: Patients with moderate to severe hepatic disease have reduced clearance; lower starting doses and careful titration are recommended. Use in severe hepatic impairment is not well-studied.


Renal Impairment: No dose adjustment is necessary for mild to moderate renal insufficiency. Data for end-stage renal disease are limited; but doxazosin is extensively protein-bound and not effectively removed by dialysis.


Clinical Considerations and Current Role



Despite the decline in its use as first-line antihypertensive monotherapy, doxazosin remains an important agent in the management of BPH. Its once-daily dosing, favorable side effect profile, and dual benefit in patients with both hypertension and BPH make it a practical choice. Moreover, the development of the gastrointestinal therapeutic system (GITS) formulation has improved tolerability. Comparative studies with other alpha-blockers (e.g., tamsulosin, terazosin) show equivalent efficacy, with doxazosin having a slightly higher incidence of dizziness and fatigue but comparable discontinuation rates.


For hypertension, doxazosin is often used as a third- or fourth-line agent when multidrug regimens are needed, particularly in patients with difficult-to-control blood pressure or those who cannot tolerate other classes. Its role in treating resistant hypertension is supported by its use in combination with agents from different classes.
