FML Forte is indicated for steroid-responsive inflammatory conditions of the eye, including allergic conjunctivitis, vernal keratoconjunctivitis, corneal abrasions with inflammation, herpes zoster keratitis (without epithelial involvement), iridocyclitis, and other forms of anterior uveitis. It is also used post-operatively to control inflammation after cataract or refractive surgery. However, it should not be used in patients with epithelial herpes simplex keratitis (dendritic keratitis), fungal infections, or untreated bacterial infections.


Dosage and Administration

The typical dosage is one to two drops instilled into the conjunctival sac every two to four hours during the initial 24–48 hours, then tapered to four times daily as inflammation subsides. In severe cases, the frequency may be increased. The bottle should be shaken well before use because it is a suspension. Patients should avoid touching the dropper tip to any surface to prevent contamination. Contact lenses should be removed before instillation and may be reinserted 15 minutes after administration.

Fluorometholone is poorly absorbed systemically due to rapid metabolism in the eye and liver. Its higher lipid solubility allows corneal penetration, but the 0.25% formulation does not significantly increase systemic side effects compared to the 0.1% version. It should be used with caution in patients with glaucoma or a history of steroid-induced IOP elevation. Intraocular pressure should be monitored regularly if therapy is prolonged (>10 days). The drug is pregnancy category C; use only if clearly needed. Nursing mothers should exercise caution. Pediatric safety has not been established for prolonged use.


Drug Interactions

No significant systemic interactions are known due to low absorption. However, concomitant use with other topical ophthalmic drugs may require a 5–10 minute interval between medications to avoid dilution.

Compared to prednisolone acetate 1% (which is more potent), FML Forte has a better safety profile regarding IOP elevation but may be less effective for severe inflammation. Compared to loteprednol etabonate (a "soft steroid"), fluorometholone has a similar IOP risk profile but is slightly more potent.
